About the position

The PSW or HCT provides personal support, homemaking services, and direct hands-on care to patients in the patient’s home. The PSW or HCT will work with the health care team members to meet the needs of their patients.

Within these programs, the PSW and HCT provide enhanced personal support to a variety of patient populations in the community, including but not limited to the pediatric, palliative, complex care, and chronic disease management. 

Requirements:

  • Must have a personal vehicle
  • Must have a current driver’s license and proof of vehicle insurance
  • Willingness to travel neighboring communities
  • Will be asked to provide or complete criminal background check

Work conditions and Physical Capabilities:

  • Attention to detail
  • Lift and carry using appropriate techniques
  • Bend, push, pull, grip, reach, kneel, crouch using proper body mechanics
  • Walk, sit, stand, climb stairs
  • Hand movements
